Health insurance for 5th year [PhD students from doctoral schools].

Doctoral students who will not be entitled to a doctoral stipend and have no other title to health insurance after 30 September 2023, and who have doctoral status, must re-register for health insurance at the university, as they will be automatically deregistered from insurance on 30 September 2023.

Health insurance is available only until such a person has the status of a doctoral student and has no other title to insurance (e.g., employment contract, contract of mandate).

It is important to remember the obligation of a doctoral student to inform the Student Welfare and Support Office UW of the need to deregister from insurance due to the fact of taking up a job (even if the contract was concluded at the UW). Failure to do so will result in “dropping out” of the NFZ system and inability to use public health care services or obtain an European Health Insurance Card. After termination of employment, remember to re-register for insurance.

To apply, submit the following documents to the BPM:

  • certificate from the doctoral school on the status of a doctoral student with an indication of the end date of this status
  • ZUS ZZA form
  • statement about not being subject to health insurance on any other account.

Additionally, foreign doctoral students should submit in paper form:

  • A copy of a valid Pole’s Card or a decision on recognition as a person of Polish origin within the meaning of the provisions on repatriation
  • A doctoral student coming from outside the EU/EFTA countries may take out voluntary insurance by signing a voluntary insurance contract with the National Health Fund and pay the health insurance premium on their own.
  • A Doctoral student who is a citizen of an EU/EFTA Member State may use public health care on the basis of an EHIC (European Health Insurance Card) issued in the country of origin. If such a doctoral student is not entitled to health insurance in the country of origin, he or she may be registered for health insurance by the university.
