- A doctoral student who is a citizen of a Member State of the European Union or a Member State of the European Free Trade Association (EFTA) is entitled to free healthcare services provided by healthcare providers who have concluded contracts for the provision of healthcare services with the National Health Fund, on the basis of the European Charter Health Insurance or – if he is not entitled to insurance in his country of residence – he/she is reported for insurance by the University on the terms applicable to Polish students.
In order to apply for health insurance, the doctoral student submits the following documents to our office:

- A doctoral student who is not a citizen of a European Union Member State or a European Free Trade Association (EFTA) Member State, recognized as a person of Polish origin within the meaning of the repatriation provisions, may insure himself voluntarily by concluding an agreement with the National Health Fund.
- Premiums for health insurance are paid by the University after the interested party submits an application for health insurance and a copy of the contract signed with the National Health Fund.
In order to apply for health insurance, the doctoral student submits the following documents to our office:

- A doctoral student who is not a citizen of a Member State of the European Union or a Member State of the European Free Trade Association (EFTA) without documented Polish origin within the meaning of the provisions on repatriation, may voluntarily insure himself in the National Health Fund.
